Participation in Our Community Acupuncture Project
Follow these simple steps to become a part of our community acupuncture project:
Step 1: Begin by downloading the two required forms. One of these forms is the Community Acupuncture Consent Form.
Step 2: After downloading, please read the Acupuncture NZ general consent form, and sign the Community Acupuncture Consent Form. Once signed, you can proceed to the next step.
Step 3: Send us a digital copy of the signed form via email to: [email protected]
Step 4: You will receive an initial MYMOP (Measure Yourself Medical Outcome Profile) questionnaire. If you can’t find it in your inbox, please check your trash folder.
Step 5: Locate the initial MYMOP questionnaire and fill it out. Identify two specific symptoms and one affected activity, assigning scores to each based on their impact.
Step 6: Congratulations! You’re now all set to join our community acupuncture project and embark on this journey with us.
